Service Notice: This clinic provides volunteer-based technology repair and support services. All work is performed on a best-effort basis. Tech4Future Minnesota is not responsible for data loss, hardware damage, or unresolved issues. Clients are encouraged to back up all data before the appointment.

Consent: By submitting this form, you consent to having your device assessed and worked on by trained volunteers under clinic supervision. You confirm that you are the owner of the device or are authorized to request service on behalf of the owner.
